Long-term Residential Care
For individuals with long-term chronic disabilities, Lakeland Health Care Center provides a full range of services in a residential setting. Working in close collaboration with social workers, recreational therapists and rehabilitation staff, Lakeland’s RN’s, LPN’s and nursing assistants provide a level of care that can only be achieved through an outstanding combination of personal and professional skills.
There are special programs for residents with dementia and memory loss. Lakeland offers a newly renovated Male and Female units for residents with advanced stages of dementia. Support is also available for family members and spouses from social services as well as our Psychology team.
Lakeland Health Care Center’s physical rehabilitation program reflects the lifestyle desired by seniors, promotes independence and encourages a rapid return to home after rehabilitation from the broad range of surgical procedures, including hip and knee replacement and amputation, as well as stroke, and other injuries and illness.
Our staff members carefully consider the age and frailty of patients and develop individualized programs that allow patients work at their own pace, enabling them to reach their full physical potential.
The interdisciplinary physical rehabilitation team includes a physiatrist (physician who specializes in physical medicine and rehabilitation), as well as physical, occupational and speech therapists, recreation therapists, a dietitian, and social workers. Staff members also fit prosthetic devices and train patients in the use of adaptive equipment.
Patients in the short-term program are encouraged to take advantage of all the amenities of the facility including movies, recreational and entertainment programs, garden, beauty/barber salon and more.
Subacute Medical Care
The Subacute Medical Care Program at Lakeland Health Care Center provides an alternative to an acute care hospital stay in the case of certain medical conditions, such as pneumonia, coronary bypass surgery, cancer, nervous-syetm disorders, cardio-thoracic surgery, vascular surgery, orthopedic surgery, stroke, as well as post-surgical, burn and other wounds.
Our program offers state-of-the-art rehabilitation and clinically complex care in a nurturing environment. The average length of stay is two to six weeks.
Whether the length of stay is several days or several months, a personalized plan of care assures appropriate medical, therapeutic and social services at every stage of a resident’s rehabilitation. Lakeland Health Care Center draws on its extensive experience to help ensure that patients are out of bed and mobile as quickly and as often as possible.
Through its Respite Care program, Lakeland Health Care Center provides temporary skilled nursing home care for the frail elderly, while providing needed respite for their caregivers. During their stay, guests reside in a warm and comfortable environment and have all their medical needs attended to by Lakeland’s well-trained staff, through individually-developed care plans which may include physical therapy, speech therapy, occupational therapy and other needed care services.
Individuals who participate in the Respite Care program may choose to eat in the dining room with other residents or in their own rooms, and are encouraged to participate in all Lakeland social and recreational activities, such as music programs, crafts, exercise, movies, outings and much more. A social worker is available to help the guest adjust to new surroundings.
